Air Travel Related Deep Vein Thrombosis
The risk of deep vein thrombosis (DVT) and pulmonary embolism (PE) may be increased by the immobilisation on journeys of more than five hours duration. The Department of Health has developed advice for passengers based on their risk of developing DVT. All passengers are encouraged to do regular flexion/extension exercises of the lower limbs, take deep breathing exercises, and walk around the cabin when safe. Drinking plenty of water whilst avoiding excess coffee and alcohol is also helpful. Travellers at increased risk include those with a history of DVT or PE, recent hospitalisation and immobilisation including major surgery especially hip or knee replacement, those suffering from heart failure, lower limb paralysis or malignancy and stroke patients. These passengers should seek medical advice before travelling. Considerations include exercises, compression stockings and anticoagulation. Amongst women, the risk of DVT is inceased in pregnancy, following delivery and in those on the contraceptive pill or hormone replacement therapy (HRT).
